You have been issued a 60 day temporary permit. One of the following options must be completed following the completion of your driver education course in order to obtain the original permit from the Bureau of Motor Vehicles.
Option 1: Student must mail the following documents to the Bureau of Motor Vehicles-Exams:
- Completed Learner’s Permit Application-MVE-64. (obtained from the driving school, Motor Vehicle branch office or online at:
http://www.maine.gov/sos/bmv/forms/
- Bottom half of driver education course completion certificate.
- Photo copy of birth Certificate or passport.
- Individual check or money order made payable to the Secretary of State for $35.
- Mail application and supporting documents to:

- If everything is in order, a learner’s permit will be issued and mailed to the student. Please allow 2 to 4 weeks for processing and delivery.
Option 2: Student may visit a Bureau of Motor Vehicles branch office or mobile unit with the following documents.
- Completed Learner’s Permit Application-MVE-64.
- Entire driver education course completion certificate; branch office or mobile unit will detach and retain the bottom portion.
- Original birth Certificate or passport.
- Individual check/money order, cash or credit/debit card for $35 for the application/permit fee.
- If everything is in order, a learner’s permit will be issued at the branch office or mobile unit.
